Forms
We have an emergency medical form that is filled out and carried with the coaches at all times in case of an emergency. We have an insurance waiver that proves the student’s family has insurance. We have a physical/OHSAA form that gets filled out by a doctor when they receive a physical. Lastly, we have code of conduct/drug testing consent form that when signed gives consent to be drug tested and states that the parent and student have read the policies and understand the information provided in the code of conduct. These are important because all forms must be completed and returned in order to participate.
